Problems during RCM

Hi,

We are facing some problems during RCM in ARIS 7.1.

1. Some models were not moving to Release DB and were still present in the Lock folders and Review DB,despite Approving and Rejecting them.

2. The status of the models in RCM Lock folder should be mentioned.

Example-  

But the status of a model was missing in Lock folder, only name was there.

What are the probable reasons behind the issues? Please help if anybody faced same problem.
